If you are unable to provide some of the required documents, there are other ways to verify your eligibility for Healthy San Francisco.
If you are unable to provide proof that you live in San Francisco, someone you live with can sign an Affidavit of Support to vouch for your physical address. They will also need to submit proof of physical address that shows their name. For example:

If you earn your income in cash and cannot provide a verification document, you may submit a Cash Income Statement to verify the amount you earn.
If you are unable to provide proof of income, you may submit an Employer Statement to verify the amount you earn.
If you are self-employed or own a small business and do not have a current tax return, you may use a Profit and Loss Statement to document your income.
If you would like appoint someone to assist with your application, communicate, and make decisions on your behalf in relation to the program, please complete the Authorized Representative Form with the person you would like to appoint.
