Schedule Appointment

To apply for Healthy San Francisco, you will need to make an appointment at one of our enrollment sites. At your enrollment appointment, you will meet with a Certified Application Assistor. Your Assistor will help you apply for the program and verify your eligibility. You will need to submit certain required documents before or during your appointment, or your Assistor will be unable to complete your application. After scheduling your appointment, you can submit documents.

You can make an appointment at the San Francisco Health Plan Service Center at 7 Spring Street by clicking here (you need to have an email address) or calling 1 (415) 777-9992, Monday through Friday, 8:30am to 5:30pm. Assistors can help  you apply for Healthy San Francisco, Medi-Cal, Covered CA, and other benefits.
